STEP 2

After you have completed the admissions steps, you need to obtain disability verification from a certifying professional and submit it to DSPS.

Documentation needed to apply: IEP, 504 Plan, Medical or Psychological documentation. You may also have a certifying professional complete the following Confidential Disability Verification form.

Intake Appointment

STEP 3

  • Schedule your appointment by calling (209) 954-5151, ext. 6272 or visit the DSPS office in the DeRicco Building, Room 110. Appointments are scheduled up to two weeks in advance.
  • Drop-ins are available for quick questions, based on counselor availability. Subject to change.
  • New students must check in 15 minutes before their scheduled appointment time to complete a pre-intake packet. Documentation must be shown to staff at time of check-in to proceed. Students who do not bring disability verification to intake appointment will be rescheduled.
