Behavioral Assessment and Referral Process

  • All campus personnel should be alert to signs of mental distress in students.  This can include (but is not limited to):
    • Shows visible signs of anxiety and/or depression
    • Lacks direction or focus (school, career, etc.)
    • Expresses suicidal thoughts or thoughts of harm to self or others
    • Shows substance abuse or addiction problems
    • Appears to need resources (food, shelter or other assistance)
    • Shows or expresses grief or bereavement concerns
    • Shows visible signs of abuse or self-abuse (cutting, burning, etc.)
    • Appears to be a victim of assault

  • File a SJDC Student Mental Health Incident Referral Form so the process to provide support will begin and the student may be contacted.

  • If the concern involves potential Title IX violations, an ACTS team member will also notify the Title IX officer.

  • A crisis counselor will be notified of the concern through the Maxient reporting system.

  • The crisis counselor will contact the student and will assess the level of concern and make appropriate recommendations for the student’s care which could include:
    • Mental health counseling through the Delta Health Center
    • Basic needs support through the Basic Needs Center
    • Financial Aid support (gas cards or other forms of financial support)
    • Outside referrals

  • Each case is reviewed by the ACTS committee before it is closed to ensure that the proper actions were taken and the case is resolved.
