Student Vaccine/Booster Upload Form
Step by Step Instructions on how to upload proof of vaccination/booster.
- Go to this link: https://hitlrte.secure.force.com/NexGen

- Enter your student ID number, last name, and Delta College student email, and then click “Record a Vaccine”
Tip: Use “Mustangs” instead of “Students” in your email address. Example: Use “abreitler486@ mustangs.deltacollege.edu” instead of “abreitler486@ students.deltacollege.edu.”
Tip: Take your time and make sure your ID and name matches how they are displayed in MyDelta.
- Once you see the green “Success” bar, check your email for an email with a secure link.
Tip: The secure link will be valid for only 15 minutes, so be sure that you are ready to upload your vaccination/booster information right away.
Tip: If you are both a student and an employee, you will need to complete this process twice using both your employee and student (Mustangs) email.
- Once you have clicked on the secure link, provide the required information for the Terms of Consent and click “Submit.”
- You’ll then be prompted to enter your vaccine information. Indicate your vaccine status.
Tip: For this process, “Fully Vaccinated” means either have you received your full initial sequence of vaccination shot(s), or you have received your full initial vaccination shot(s) and your booster. If you received only one of the two initial shots, you are considered “Partially Vaccinated.”
Tip: Vaccination information that you have previously submitted to the College should appear automatically.
- You’ll be asked to provide the vaccination country, manufacturer of your vaccine, and the date of each dose. You may enter the lot number but it is not required. Below those fields, click “Choose file” to upload either a photo of your vaccination card with all of your vaccination/booster dates, or upload a screenshot of your California Digital COVID-19 Vaccine Record, which you can find at https://myvaccinerecord.cdph.ca.gov
Tip: You can upload more than one file if needed.
- Check the box under “Attestation and Consent” and click “Submit.”

- Once successfully submitted, you will see green “Success” bars. A confirmation email will be sent to your Delta College email letting you know that the information you submitted is pending review. This does NOT mean you have complied with the mandate.
Subject line will be: Your COVID-19 vaccination/booster has been received - Once your vaccine information has been verified, an approval confirmation will be sent.

Here is how to apply for a medical or religious exemption.
Reminder: If you already have an approved medical or religious exemption from the original vaccination mandate, you do not need to apply for another one.
- Visit the Student Vaccine/Booster Exemption Request Form
- Enter your student ID, name, email, and date of birth.
Note: Please use your new “Mustangs” email address. For example, abreitler000@ mustangs.deltacollege.edu rather than abreitler000@ students.deltacollege.edu. - Under “Vaccine Mandate Status,” select “Exemption Requested”
- If you are seeking an exemption for the booster mandate only, check the first box
- Select either a medical or religious exemption
Exemption Form Example
- Check the remaining boxes acknowledging that you understand that submittal is not a guarantee of approval, that exemptions may take up to 14 days to process, and that twice-a-week testing is required for those with approved exemptions.
- Click submit
- You will receive a confirmation email that your exemption request has been received. This does NOT mean that your exemption has been approved.
- You will receive a second email once your exemption is approved. You will then be able to sign up for in-person or hybrid courses. You may also receive an email seeking more information in order to process your exemption request.
- You will receive email instructions on required twice-a-week testing at the Locke Lounge.
