Important Notice to All Students:

Completed Application packets must be submitted to

Holt 140 BEFORE 5:00 pm, Friday, Janauary 29, 2010.

Faxed Student Application Packets will NOT be Accepted

If the application packet is printed from this website, the student MUST pick up an INSTRUCTOR ADD FORM from Holt 140, Applied Science & Technology Division office and submit the form with your application packet to be considered a complete packet.

Complete application packet includes the following:

1.  Student Agreement form (both pages with signature)

2.  Application form

3.  Employer Verification form

4.  Instructor Add Memorandum form

(Required information: Student name, Delta ID, Student signature and date)

All Student Responsibilities must be completed by Friday, January 29, 2010 and the submission of your application packet. 

All application packets are subject to review and can be rejected if student has not completed the student responsibilities.

All application packets must be neat, clear and

complete for review.

Application acceptance & student participation pending Internship/Work Experience Instructor availability.
