Work Experience Student Application Packet

Notice to All Participating Students

Application acceptance & student enrollment is pending Work Experience Instructor availability AND student employment duties falling within the designated areas.

Summer 2013

The program will not be available until the Fall Semester.  Please check this site in July for additional information.

Important Student Participation Notification

Due to the diminished number of resources (courses offered, Work Experience Instructor availability), students who are interested in participating in the Work Experience Program must be aware of the following requirements:

 
  1. The participating student must be employed with an organization either as a paid or non-paid employee prior to submission of Student Application Packet.

The student must find his/her own employment.

This program is not Federal Work Study.

  1. The participating student must be enrolled in a SJDC course or courses for the participating semester, including late starting courses. The student must be on the officially enrolled in the course; the wait list does not meet the requirement.

        Your employment duties must fall within the designated areas

If the student has any unpaid fees or registration issues which would delay enrollment into the program, Student Application Packet will automatically be disqualified.

If a student is unable to complete the average number of hours per week, the student will not earn the unit(s) requested from the program. Please review your employment schedule prior to submission and discuss the requirement with your supervisor.

Print Student Application Packet

PRINT:  Work Experience Student Application Packet - Not available for Summer 2013

Student must come to Division Office to complete the Instructor Add Memorandum.  The document will not be mailed, faxed, or e-mailed to the student.

Application Packet contains form fields, you may save the document to your desktop and complete information prior to printing.  You will need Adobe Acrobat Reader to complete the application packet on your computer.

Important Notice to All Students:

Student Application Packets must be submitted and approved

before deadline.

Faxed Student Application Packets Will NOT Be Accepted

The student will complete an INSTRUCTOR ADD FORM in Holt 140, Applied Science, Business & Technology Division office when application packet has been reviewed and approved at the time of submission.

Complete application packet includes the following:

  1. Student Agreement form (both pages with signature)
  2. Application form
  3. Employer Verification form
  4. Instructor Add Memorandum form (acquire from division office, Holt 140)
    (Required information: Student name, Delta ID, Student signature and date)

All Student Responsibilities must be completed prior to the submission of your completed application packet. 

  • All application packets are subject to review and can be rejected if student has not completed the student responsibilities.
  • All application packets must be neat, clear and complete for review.
