Use this form to update the Office of Student Activities about changes to your executive officers and/or ICC representative.

Use this sheet to record the number of items and/or tickets sold during the fundraiser

Meeting Times

Use this form to let the Office of Student Activities know when your club is meeting

Club Event Registration

Use this form to let the Office of Student Activities know about an upcoming club event

ASBG Grant Application and Funding Request Bylaw

At this time the ASBG is not accepting grant applications for the 2012-2013 academic year.

Temporary Event Application

This packet is required by the City of Stockton anytime your club or organization host an event outside where food will be cooked and served from a temporary food facility. The form must be submitted two weeks in advance.

W9

All persons receiving payment for services provided to the College must have a W9 on file with the Business Office before a payment can be processed.
