To request Transcript Evaluation through In Person or By Mail, please download and complete the Transcript Evaluation Form and submit:

  • In Person - Admissions & Records Service Window located in the DeRicco Student Services Building. Monday-Thursday 9am-4pm, closed Friday OR
  • By Mail -
