*IMPORTANT INFORMATION for SPRING 2010*

This information is to inform you of the EOPS/CARE Program requirements and deadlines that you will need to meet in order for you to be approved to receive EOPS/CARE services for the Spring 2010 semester.

You will need to provide the following by 12:00 p.m., NOON, Friday, February 5, 2010.  Your failure to comply with this deadline will prevent the CARE Program from providing you services in a timely manner.

CARE Requirements and documentation:

Complete and return the renewal form.

Have an SEP (Student Education Plan) on file for the Spring 2010 semester.

Submit a Passport to Services showing January-May eligibility.

Submit a copy of CalWORKS Welfare to Work contract if you are in the CalWORKS Welfare to Work Program.

You must be enrolled in at least 12 units of approved SEP courses by the February 5, 2010 priority deadline.

Be in compliance with the EOPS/CARE Programs.

You must have passed the Guidance 18C class (to be eligible for CARE grants).

You should have a completed FAFSA on file with Financial Aid showing unmet need.

Must be enrolled in Guidance 18A CARE class for Spring 2010.

Submit all forms to the EOPS/CARE Office, DeRicco Student Service Building - Counseling & Special Services, 2nd Floor.  Hours of operation are Monday - Thursday, 8:00 a.m. - 5:00 p.m. and Fridays, 8:00 a.m. - 12:00 p.m.
