Policies and Procedures



Obtaining Services/Program Requirements

 

1.Participation in DSPS is voluntary. If a student wishes to take advantage of the support services and formal academic accommodations provided through DSPS he/she must contact the DSPS office and schedule an appointment to meet with a counselor.

 

2.In consultation with the counselor, the student will complete an intake interview, a Student Educa-tional Plan and sign a Consent for Release of Information form.

 

3. In order to be eligible for services provided by DSPS, the student must have a disability which is verified by an appropriate professional and which results in an educational limitation. The student will be responsible for providing appropriate documentation of his/her disability. Eligibility for DSPS services will be determined in a timely manner.

If the student is deemed ineligible for program services and wishes to appeal the decision, the student must follow District Student Grievance Procedure (District Policy #5460). Your counselor will provide a copy of these procedures upon request.

 

4.The student and counselor will work together to determine the student's educational needs. The counselor will authorize any reasonable and appropriate support services and/or academic accommodations which the student may require for academic success. Any services or accommodations recommended must be directly related to the educational limitations which result from the student's verified disability. (Please refer to Services and Accommodations listed in the next section.)

 

5.Students should make their requests for services in a timely manner. Consideration should be given to the nature and type of accommodation being requested.

 

6.If the student is dissatisfied with the services provided by DSPS, denied academic accommoda-tions or the use of auxiliary aids by college faculty or staff, the student should immediately schedule an appointment with a DSPS counselor or the Director of the department. If the issue can not be resolved to the students satisfaction, the student should follow District Student Grievance Procedure (Policy 5460). DSPS will provide a copy of the policy upon request.

 

7.The student's records with DSPS are confidential in nature. They are kept separate from other educational records and will not become a part of the student's official educational file.

 

8.Students receiving services through DSPS or enrolled in specialized instruction shall not be precluded from participating in any other course, program or activity offered by the college, provided all prerequisites and requirements are otherwise met.

 

9.Students should be responsible in their use of DSPS services and follow policies and procedures established by the department.

 

10.Students should meet academic standards established by the college and make measurable progress toward the goals established in their Student Educational Plan.

 

11.DSPS will respond in a timely manner to reasonable requests for support services and academic accommodations.

 

12.Unfortunately, DSPS is unable to provide services of a personal nature (e.g., personal attendant care). However, there may be other resources available on and/or off campus to assist with these needs.
