Test administration or proctoring an exam are services which Disabled student services provides in order to (1 assist the student in getting his/her testing needs met and 2 to minimize any problems you may encounter in relation to the student's unique needs).
Testing physically or visually limited students may require some degree of adaptation. The areas checked below describe the services that this student may need to function successfully in the testing process:
If you are interested in using these services, we need the following: _____the testing information form filled out indicating HOW you want the test administered (time limitations, open/closed book; etc.) and WHEN you want the test administered. The guideline we follow is that the test must be given the exact day and time that the other students in the class are taking it, for obvious reasons. If a student needs additional time related to his/her disability and limitations, we may need to find a block of time (on the same day) that fits into the student's schedule. If a student misses the scheduled test day/time, we will not reschedule without your O.K.
_____the actual TEST with TESTING INFORMATION FORM attached should be delivered (or sent by E- text, on computer diskette or campus mail) so that it arrives in advance of the testing day. The test is kept in a locked file until the student arrives for the exam. We allow only our non-student staff to handle these exams – student helpers are NEVER allowed access to exams. Please note; that it is the student's responsibility (not yours), to contact this office one week prior to the test date to request test administration or proctoring assistance. Also, if you give short pop quizzes in the class, please contact us at your earliest convenience to discuss how we can assist you and the student.
