How To Obtain Services

For Elevator Key

Student:

• Need to bring:

          DSPS Disability Verification form - (Printable) (Text)

          Student Data Form for Elevator Key/Student Education Plan - (Printable) (Text)

• Student must first pay a $3.00 deposit at the cashier’s window and bring their receipt to Danner 109, the health unit, where they will fill out some paperwork and receive a key.

Staff:

• Staff need to contact the key dept.

Wheelchairs/Scooters

If a student with a disability needs access to wheelchairs/scooters, they will first need to register with DSPS, then come to Danner 109 and go over the rules and operating instructions with a specialist.

Mobility

To obtain mobility assistance the student should either contact DSPS or the health unit. After contact we can pick student up from bus stop or they can be dropped off on campus or in front of the health unit.

Dial-A-Ride and the Hopper drop students off near our office. A student may call our office at 954-5078 or pick up a campus blue phone and ask for assistance.
